I don't think that's quite right. The HEAD _element_ is required,
but neither start nor end tags are required:

--- begin quote ---
<!ENTITY % html.content "HEAD, BODY">

<!ELEMENT HTML O O (%html.content;) -- document root element -->
--- end quote ---

The TITLE element is also required:
