What are you setting the action attribute to? Should be the path to your
script.
If the script is in the same directory as your form:
<form action="script.php" method="post"></form>
or, with a path absolute to the web document root:
<form action="/path/to/script.php" method="post"></form>
or, with an absolute URL:
<form action="http://example.tld/path/to/script.php"
method="post"></form>
