The WHERE and ORDER BY clauses should be:

WHERE utbl7.fld5 >= @fld1val
OR (utbl7.fld5 = @fld1val AND utbl7.Unique_ID > @uniqueid)
ORDER BY utbl7.fld5, utbl7.Unique_ID
