If the third party server is running a web-server, allows upload on php, then
you can have the receiving php-script on the third party server and the form
on your default web-server. You may want to make a redirect back to the
default web-server from the third party server.

If the third party server don't have a web-server (your you can't use your
upload script there), then you need to transfer the uploaded file on some
other way as ftp, keep in mind that you will increase the bandwidth usage with
this method.
