I have yet to see a web host that did not have mod_rewrite already installed, so
that's not usually a problem.

To rewrite requests only if the filename doesn't exist, use this:

RewriteEngine on
RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteRule ^(.*)$ index.php?username=$1 [L,QSA]
