Test this:

test-include.php:

<?php
$test="'test-include.php' has been included";
?>

Your test script:

<?php
include_once('test-include.php');

print "<p>$test</p>";
?>

If output of yout test script ist not: <p>'test-include.php' has been
included</p>, then test-include.php hast not been included, but I dont
think so.
