I support a software product written in VBA (Microsoft Access). Customers
contacting us for support rarely tell us enough information about the
software (version, release number etc) .

I would like put a link in our sofware to a web page (I can do that part) so
that when the user clicks on the link it automatically puts the version,
release number etc on the web page ( i.e. it pre-fills some of the fields on
the web page) . This part I don't know how to do.

After they fill -in more information I want to use PHP to send me an email
of the web page data to me. I can do this part.
