Any PHP script you find would allow you total control of the appearance
of the form; PHP isn't about appearance.

You want a script which will take the $_POST['...'] information for the
two form text boxes and write it to a database. Like you said, this is
a really simple script, and while you can probably find a script for
what you're looking for somewhere, I think it would be far more
beneficial for you as a 'PHP newbie' to try building it yourself.

Also, if you're going to develop in Dreamweaver, make sure you
http://validator.w3.org/ validate your HTML.
