My personal best recommendation is "PHP 5 / MySQL Programming for the
Absolute Beginner" published by Thompson. It starts off quite basic and gets
into reasonably advanced topics. I found it more useful as a reference and a
"get familiar" type of book - I would suggest looking through it first, but
it worked rather well for me.
