Don, if I were you I'd start with an MS Access or MS Excel form to
actually take the surveys. Then I'd keep the file on a shared
folder....when the user who took the surveys gets back into the office
(where there is a connection to a master db) I'd give them a button to
upload their survey(s). Or else you could have the master db look for
new files on the share and automatically upload the data.
