I guess your page is called something.html or something.htm, you need to
rename the file to something.php before the server will pass the file to php
(and only if the server has php enabled).

If you have done it right that far, a good option is to post your code, so
people can check what is wrong.

If the page which your form-action points at is a php script, there is no need
to send the IP in the form.
