Try this for the top link:
<li><a href="#1"><b>Text for description</b></a></li>

And this for the destination:
<p><a name="1"></a><b>Q:</b> Text for block<br />
<b>A:</b> More text<br />

And finally this to return to the top of the page
<a href="#id of top div or link as the first one"><i>Back to Top</i></a></p>
