I have it working on Windows, Apache, PHP5
A few checks you could make:
Are you sure that var_name is the name of the file upload field in your
form?
Have you set the form enctype to "multipart/form-data"?
Have you set the maxfilesize attribute, and does the file you are trying to
upload exceed that size?
