Very few people are blind AND deaf. But can still use
braille interfaces.

That's exactly the point. Unless your site is intended for
an english speaking, able bodied audience, anybody in the
world should figure it out. It's called accessibility.
My idea of "image+alt attribute" is the best I've come up
with, but there's for sure some smarter way to do it.
All the javascript suggestions are interesting, but it
doesn't take much to execute the code and reap the email,
just like the dumbest browser does.
