Same here in IE6, Win2k.

Exactly - even when accessing the image directly it displays the empty
box.

FWIW, it works fine in IE5 and 5.5. My guess is that a recent Windows
update may have broken it - possibly the fix for the .wmf vulnerability
(though your image is a gif). Other than that I have no idea, sorry.
