server = mysql
The problem has nothing to do with your apache, it's just the "user" who
executes php. You need to configure your mysql to not use authentication and
your problem is gone.
If you want things to work out of the box without any problems, then install
an OS that includes apache, php and mysql as part of the applications shipped
with the OS.
