Maybe a bit naive, but:
new image = > new name??????
Why does do they have to have the exact same name, other then it's easier to
just overwrite the image instead of doing the work?
You could send cache headers while creating a dynamical image, or meta tags
on a HTML page. Users/browsers decide wether to cache or not though, all you
can do really is "advise" them.
