Can someone please tell me the codes or how to put headers up with a layout?

Cause I'm in alot of communites that make simple colored or just striped layouts they're cute and all. But, I got some nice headers to go that would go well with them. And I'm able to custimize they layouts with the headers. But I don't know how to.

So yeah..how do you add a header in your journal?
